You are automatically enrolled for single coverage under the benefits plan after one month of continuous active employment. You have 31 days from that eligibility date to enroll your immediate family members. Forms will be mailed to you or downloaded below. If members are not enrolled within that time frame your eligible dependents will be deemed as late applicants and must complete Proof of Good Health form and be approved by the insurance company in order to obtain coverage.
- Complete all sections of the enrollment form online and submit electronically. This will initiate your enrollment.
